There are a couple of places where they do not make it clear how to move on without accepting their MAX or other extra charge. More explanation on the health insurance question. My health insurance is via my retirement (employer) I wish you would have just had a question that said that as I got no form for it but I know it comes from the Employee Retirement System of Texas.
