Missing step about Expiration and Issue dates

There is NOT a clear instructions when entering your State Driver's License, after MM/DD/YYYY you must also enter your Expiration and Issue dates.
Right below the information about not having a State DL or ID card it has a box and to the right of the box it reads example123456 It does NOT read to enter the Expiration and Issue dates. It won't let you continue until you enter the dates.
