THE PROGRAM WAS DIFFICULT TO USE AND FREQUENTLY REFERRED THE USER BACK TO ALREADY FILLED IN SCREENS. IT ALSO DID NOT RECOGNIZE ITEMS CHECKED WHEN INITIALLY ENTERED.
