Generally just the item with all parts and accessories and your receipt! They can look it up if you are a rewards member and/or if you used a debit or credit card. Just have to bring the card in physically to swipe through to pull up your receipt. If you are a rewards member, just give them your phone number and it can be looked up. As long as that info was put on the original receipt. Item can be opened. If you tried it and didn't like it, No problem. Just make sure it is still in good condition and within the return period which is the least 15 days (depending on your rewards membership) Some members get an extended period of 45 days.
