Template does not line up properly when printed

Doesn't line up right when printed. Top left is lined up, but as you go diagonally, down and right, everything gets progressively further off template.
I'm using a Canon MF642C this is not a cheap printer, nor one I've ever had any other issue with.
No, I would not recommend this to a friend.

We are sorry you had trouble printing your project. Here are some general settings we would recommend checking. Be sure that the page size is set to 8.5 x 11 or US Letter and that the Scale is set to 100% or Actual Size is selected. You can usually find this option on the print dialog box. We would also recommend selecting a thicker paper type such as Label or Heavyweight.
