When I bought the MX Master 4 I did not realize that my OS version is too old to run the Logi Options+ configuration software, so I cannot yet make use of the bottom four buttons and cannot yet flip the direction of the horizontal scroll (I wish the default were reversed because 90% of the time I expect it to go in the opposite direction) until I get around to updating my OS (this mouse is an impetus, lol).
I never owned a 3S but I did test one out at a local store and did not like it because a)its horizontal scroll wheel was too deeply recessed to be of any use and b)the clicks were annoying and c)the weight of my fingers seemed to trigger clicks, but that might be only because I was standing up. So I decided to get a 4 instead.
It is definitely a heavy mouse, but I find I can lift it without pressing any buttons by placing my thumb just underneath the row of three narrow buttons, although the surest grip is by the bottom, albeit unbalanced. It really could use some sort of "grip ring" for ergonomic lifting, as other reviewers have noted. Although the weight makes one psychologically afraid it'll drop, I have never dropped mine while holding it aloft. Thanks to the high resolution I rarely need to lift it anyway, and I only have a 6x8" area for my mouse and wrist. NB: I am not a gamer.
Ergonomically it is off-axis for me (see photos). The bump at the lower left behind the giant gesture button (circled in light blue) most naturally wants to go to the center of my palm because otherwise it competes with the convexity of my thumb muscle. In future I think that region should be made concave if Logitech expects fingers to be parallel to the left/right buttons instead of at an angle as mine naturally go (see second pic). I hold the mouse with my palm resting on the desk because that is the most comfortable, relaxed position. And I'm happy that my finger weight does not click the buttons from this position. Oh, I am a person of medium height.
The horizontal scroll wheel, while physically vastly better than on the 3S, does not scroll smoothly and therefore 80% of the time I just use the vertical wheel with a modifier key because it's better control.
Speaking of the vertical scroll, I keep mine set to smooth scrolling since the e-detents were too chunky. I love the "infinite" scroll! So helpful to get to the end of a long-@** menu, lol. Wheeeee!
One of the negative reviews spoke of sharp edges around the scroll wheel, but it's smooth on mine so they must've received a manufacturing error. I couldn't scratch myself on it if I tried, lol.
Oh, and I love the quietness of the clicks! "It's like buttah." The sensory feedback through my finger is completely sufficient. A few reviewers said they couldn't feel it, but either their mouse or their finger is defective because I feel it just fine.
Therefore I give this mouse 4 stars instead of 5 only because there is still room for significant improvement (and because I cannot fully evaluate it until I update my macOS). I am SO glad I got this one and NOT the 3S (thanks to my sister for making sure I was truly sure before ordering, which led me to go to a local store to try the 3S).

I like it almost as much as my MX 3. My only complaint is that it's a little harder to pick up and move because the sides are not as deep and the rubber coating isn't as grippy. It does seem that adjusting the volume with the thumb roller is working much better though.
